My roomate has a shih tzu puppy that is about 3 months old. Every time she takes her out of the crate to take her to use the bathroom outside, she pees on herself. We both try to approach her in a "loving" way, but that doesn't work. When she is at work, I have to take care of her puppy and it drives me crazy. We can't open the crate and let her come out by herself because she will still pee, so that doesn't really make much difference. What can we do so that she does not pee every time we go to take her out of the crate?
Sounds like this is a nervous pup. The breeder may have had a hand in that. Just try to be calm when there are accidents. Over time this may get better. I had a dog that everytime I went to pet her, she would drop and pee. We think the breeder's kid was hitting her on the head. It was bad, but over time she gained trust and quit. Make sure if he/she does make it outside, give treats and praise.
When you get her out of the crate, don't say anything to her. When you do speak to her, do it in a low tone. Don't talk in a high pitch voice as this will just make her more excited and she will pee more.
