Some dogs develop an OCD about areas on their bodies to lick. So much so that they cause what's known as a lick granuloma which can look like a wound cause they lick it so much it becomes irritated like a sore. If he is licking off the medication and chewing off the bandaging then your only other option is to buy an E-Collar at the pet store and have him wear that until the wound is healed. your dog will hate the collar very much but sometimes we must be a little tough on them for their own good. If wearing the collar turns out to be a disaster, then I would put a leash on him, clean and medicate the wound with a topical product from your vet, and them immediately take him for a walk. Taking him for a walk right after applying the topical medication will *hopefully* keep his mind off the wound for a long enough period of time for the medication to work.
Aas far as him being thin. German shepherds are prone to all sorts of digestive issues. A main problem with them is Irritable Bowel Syndrome and Pancreatic Insufficiences. They also can be terrible eaters. A vet visit would be a good idea to determine why he is so thin. It could be something as simple as worms also.
As far as his hair falling out if that is due to routine shedding, this time of year they shed more than any other time, brushing him out every other day will help minimize the amount you see all over your house.
