It sounds like he's learned what the leash is, any time he is off the leash he's going to run. I'm not sure that after all this time there's anything you can do to stop him, you may just need to keep him on the leash at all times unless he is in a fenced area. What you can try is taking him to a lot of different locations and going over the commands with him every single time. For like a year. No off leash time at any of those places. Then after that year when he responds every single time, try taking him off the leash and trying the commands. Maybe in that year time period he will have learned that come means come regardless of the location. With training puppies, the mistake many people make is that they work so hard on training the puppy, but don't train it everywhere else. So they KNOW that at home the dog knows come, sit, stay, etc. Then they take puppy to a completely different environment, like the park, and take him off the leash expecting him to remmeber the commands and he doesn't, because it's a new place that isn't home. When training, you have to reinforce all commands on the leash everywhere, so the dog will know commands and listen everywhere, not just at home.
